Code 12 = Flame failure during operation. Burner lit successfully but flame dropped out before the draw ended. Distinct from Code 11 (no ignition at all).
Diagnostic sequence
1. Flame rod cleaning (the #1 cause)
The flame rod accumulates oxidation over months/years of operation. The unit lights, the rod's signal degrades, the controller misreads it as "flame lost," and shuts the burner down for safety. Cleaning the rod with fine sandpaper resolves 60% of chronic Code 12 cases. See flame rod page.
2. Condensate blockage (condensing models only)
If the condensate trap or drain line is blocked, condensate backs up into the burner area, disrupts flame detection, and throws Code 12. Symptom: Code 12 paired with Code 25. Fix: clear the drain. See maintenance guide.
3. Vent restriction
Birds nest in the exhaust termination, ice forms over winter, or the vent is undersized. Restricted exhaust pushes flame characteristics outside the detection envelope. Inspect the termination.
4. Gas pressure drop under load
Code 12 often appears when another gas appliance fires mid-Rinnai-draw (furnace cycle, range burner). Gas pressure drops below the 4" WC minimum, flame goes weak, sensor misreads, shutdown. Fix: have the gas utility check incoming pressure or upsize the gas line. See install requirements.
5. Heat exchanger scale
Heavy scale changes burner dynamics — pre-mix burns differently when fewer GPM are flowing through a constricted exchanger. Run a descale.
6. Combustion fan (less common)
If the fan motor is failing intermittently, combustion air drops, flame characteristic shifts. Often paired with Code 79.
What does NOT cause Code 12
- Igniter — igniter is only relevant for ignition (Code 11); once flame is established, igniter is no longer in the loop
- PCB / control board — almost never causes Code 12 alone (usually a different code)
Reset procedure
- Inspect and clean the flame rod (10 minutes)
- Power cycle the unit
- Monitor for 24 hours under typical draw patterns
- If code recurs, escalate to condensate / gas pressure / vent inspection
